We serve vulnerable adults experiencing or at risk of homelessness who face complex health challenges. Our residents include individuals leaving hospitals with nowhere safe to recover, older adults with chronic conditions, and people managing serious mental health or substance use disorders. We also support those transitioning from jails, psychiatric facilities, or nursing homes who need help returning to community living.

Many have been cycling through emergency rooms, hospitals, and the streets without a clear path forward. The people we serve typically face multiple barriers to stability including disabilities, chronic illnesses, or histories of trauma. By focusing on this high-need population, we provide the intensive support needed to help our community's most vulnerable members achieve lasting independence.

Our Services

Our comprehensive services work together to heal, stabilize, and empower each resident on their unique journey from crisis to community.

  • Our Transitional Housing Program provides a critical bridge between crisis and stability, offering 30-90 days of safe, supervised housing for individuals with nowhere else to turn. This isn't just temporary shelter; it's a comprehensive program designed to address the root causes of housing instability while building a foundation for permanent housing success.

    Every resident receives personalized case management focused on their unique housing goals. Our case managers work one-on-one with residents to navigate complex housing systems, prepare applications, gather required documents, and connect with landlords willing to work with our population. We maintain relationships with local property managers and housing authorities to increase placement opportunities.

  • Our Medical Respite Program serves individuals who are too ill or frail to recover from a physical health crisis on the streets, but not sick enough to remain in the hospital. This program fills a critical gap in the healthcare continuum, providing a safe, healing environment where vulnerable adults can fully recover without the risk of complications from living unsheltered.

    We coordinate closely with hospitals, clinics, and primary care providers to ensure continuity of care. Our trained staff provides round-the-clock supervision and support, helping residents manage complex medical needs while working toward housing stability. This integrated approach significantly reduces hospital readmissions and emergency department visits.

  • Our Behavioral Health Support services address the complex mental health and substance use challenges that often accompany homelessness. We recognize that untreated behavioral health conditions are both a cause and consequence of housing instability, which is why we provide integrated, trauma-informed care that meets people where they are in their recovery journey.

    Our approach combines professional clinical services with peer support, creating a community of healing within our facilities. We work closely with County Behavioral Health and community providers to ensure residents have access to the full spectrum of mental health and substance use services they need, both during their stay and after transition.

  • Many of our residents have experienced long periods of homelessness or institutionalization that have disrupted their ability to maintain independent housing. Our Life Skills & Tenancy Support program rebuilds these essential capabilities through practical, hands-on training that prepares residents for successful independent living.

    We focus on both the concrete skills needed to maintain housing and the soft skills necessary for community integration. Our curriculum is individualized based on each resident's needs and goals, ensuring they leave our program with the confidence and competence to maintain stable housing long-term.

  • Our Community Reintegration program supports individuals returning to community living after extended periods of incarceration, psychiatric hospitalization, or long-term care. We understand that these transitions are particularly challenging, often involving rebuilding relationships, navigating new systems, and overcoming significant stigma.

    This program provides comprehensive support to address the multiple barriers faced by justice-involved individuals and those exiting institutions. We work to rebuild social connections, develop employment readiness, and ensure access to essential services while providing a supportive environment that promotes successful reintegration.
