Our primary goal is to stabilize each resident's health and housing within 30-90 days while building the skills and connections needed for long-term success. We strive to reduce emergency healthcare utilization by 50% among our residents, successfully transition 80% into permanent housing, and ensure every individual leaves with active benefits, healthcare connections, and a sustainable support network. Through partnerships with CalAIM providers and community organizations, we're working to create a seamless continuum of care that transforms crisis interventions into lasting solutions for California's most vulnerable populations.
