We aim to break the devastating cycle of homelessness and institutionalization by providing comprehensive transitional support that addresses root causes, not just symptoms. Our primary goal is to stabilize each resident's health and housing within 30-90 days while building the skills and connections needed for long-term success. We strive to reduce emergency healthcare utilization by 50% among our residents, successfully transition 80% into permanent housing, and ensure every individual leaves with active benefits, healthcare connections, and a sustainable support network. Through partnerships with CalAIM providers and community organizations, we're working to create a seamless continuum of care that transforms crisis interventions into lasting solutions for California's most vulnerable populations.
Our Mission
Buena Vista Manor ARF provides compassionate transitional housing and comprehensive support services to individuals at risk of homelessness and institutionalization, empowering them to achieve housing stability, health, and independence through person-centered care that addresses the whole individual. We break the cycle of crisis by offering safe haven, integrated health services, and pathways to permanent housing, ensuring every person we serve has the opportunity to rebuild their life with dignity and become a thriving member of our community.
